The mission of the Foundation for Healthy Communities is to improve health and health care delivery in New Hampshire. The Foundation is a non-profit partnership involving hospitals, physicians, health plans, home care agencies and other organizations working to improve health in our state.

ACP Education Programs

We will offer four training sessions in September to review updates to the ACP Guide since major changes were enacted into law in 2007 and answer questions from those who help people to think about their future health care decisions.

Who should attend?

Social workers, nurses, pastoral care staff or others who work with patients and families to think about advance care planning and complete advance directives.

Schedule:

  • Concord - Thursday, September 16: 9-11 am
    Foundation for Healthy Communities, 125 Airport Rd., Concord
  • Nashua - Thursday, September 23: 12:30-2:30 pm
    St. Josephs Hospital (Carl Amelio Room), 172 Kinsley St., Nashua
  • Whitefield - Tuesday, September 28: 1-3 pm
    Weeks Medical Offices (Conference room in back) 8 Clover Lane, Whitefield
  • Concord - Wednesday, September 29: 1-3 pm
    Foundation for Healthy Communities, 125 Airport Rd., Concord
