The mission of the Foundation for Healthy Communities is to improve health and health care delivery in New Hampshire. The Foundation is a non-profit partnership involving hospitals, physicians, health plans, home care agencies and other organizations working to improve health in our state.

The New Hampshire Hospital Association (NHHA) and the Foundation for Healthy Communities (FHC) have converted to a new phone system. Effective July 28th, you will be able to reach individual staff members by calling them on their direct dial phone number (see list below). You can also still reach us by our main phone number at (603) 225-0900. When you call our main number you will be given the option to find each of us by a directory listing, but you will always have the ability during regular business hours to opt out to speak to one of our staff members directly to help direct you.
